ChatGPT Desktop Oops Error After Second Folder
When Windows ChatGPT or Codex Desktop shows Oops, an error has occurred after a second project folder, trim that project’s rootPaths to one path.
Quick answer
Match Oops, an error has occurred on Windows ChatGPT or Codex Desktop with Update ChatGPT and Try again, after a project that worked with one folder starts crashing once a second folder is added. Fully quit the app, back up %USERPROFILE%\.codex\.codex-global-state.json and the matching .bak file, reduce that project’s rootPaths array to exactly one path, save valid JSON while the app is closed, then relaunch. Prefer that surgical edit over deleting the whole .codex folder. Keep projects single-folder until you are on a fixed build. This is not a generic chat Oops, a sign-out Oops, or a ChatGPT 429 banner.

What we're seeing
- The exact dialog is Oops, an error has occurred, with Update ChatGPT and Try again on Windows ChatGPT or Codex Desktop.
- Independent July–August 2026 first-hand openai/codex issues and an OpenAI community thread quote that dialog after adding a second folder to a project that previously opened with one root.
- First-hand leftovers edit .codex-global-state.json so the affected project’s rootPaths list has exactly one path, and they edit the .bak copy the same way because the app can restore from it.
- Some first-hand reports also mention a process is not defined stack in AppRoutes for the same multi-root trigger; the try-today leftover remains trimming rootPaths, not waiting only for an update.
- Do not blend this with VS Code Codex multi-root crashes, generic in-chat Oops waves, or unable-to-load-conversation errors.

Fix 1
Confirm Oops after a second folder
Match the Windows Desktop Oops dialog and the multi-root trigger before editing global state.
- Read the dialog. This guide is for Oops, an error has occurred with Update ChatGPT or Try again on Windows ChatGPT or Codex Desktop.
- Confirm the project worked with one folder and started failing after a second folder was added, or that the project stores more than one path under rootPaths.
- If you only see a rate-limit, login, or in-browser chat Oops with no desktop project folders involved, stop here.
Applies to Windows, Desktop
Fix 2
Fully quit and back up global state
First-hand fixes require the app closed so it cannot rewrite .codex-global-state.json on exit, plus backups of both the live file and .bak.
- Fully quit ChatGPT or Codex Desktop. Confirm no leftover processes remain in Task Manager.
- Copy %USERPROFILE%\.codex\.codex-global-state.json to a safe backup name.
- Also copy %USERPROFILE%\.codex\.codex-global-state.json.bak if that file exists.
Applies to Windows, Desktop
Fix 3
Reduce rootPaths to one path
July–August 2026 first-hand reports cleared the permanent Oops dialog after the affected project’s rootPaths array was reduced to a single folder path in both the live JSON and the .bak file.
- Open the backed-up .codex-global-state.json in a text editor.
- Find the affected project under local-projects and inspect its rootPaths array.
- Keep the original first root path. Remove the added second path so rootPaths has exactly one entry.
- Apply the same rootPaths edit to .codex-global-state.json.bak if present.
- Validate that both files are still valid JSON, save them, and only then relaunch the app.
- Do not delete the entire .codex folder for this leftover.
Applies to Windows, Desktop
Fix 4
Update and keep projects single-root
Some first-hand issues closed after later desktop builds, but recurrence threads still recommend single-root projects and a Store update after the surgical edit.
- After relaunch succeeds, check Microsoft Store or the in-app Update ChatGPT path for a newer desktop build.
- Until you confirm a fixed build, keep each project on one folder instead of adding a second root.

Device notes
- Windows
- This fingerprint is Windows ChatGPT or Codex Desktop with .codex-global-state.json under your user profile.
- Mac
- Not the validated first-hand cluster for this exact Oops-plus-second-folder leftover.
- Desktop
- Scope stays on multi-root desktop projects. Do not use this guide for ordinary web chat errors.
Official information
Checked on 14 September 2026, openai/codex GitHub issues and an OpenAI community thread document the Oops, an error has occurred dialog after multi-root projects, including surgical edits to .codex-global-state.json. OpenAI Help does not publish a dedicated article that names only this second-folder desktop crash.
